What to Bring
Upon arrival, the Social Services Director will have paperwork which will need to be signed by either the patient/resident or legal representative. Please provide the following documents:

* Advance Directive/Living Will
* Power of Attorney
* Medicare Card
* Medical Insurance Cards
* Medicaid Coupon
* Social Security Card

This is a suggested list of items you may want to bring with you. Please label with your name, if possible, prior to admission.

* Washable (no dry cleaning)
* Cotton/poly sweat clothes
* Tennis Shoes or rubber soled shoes
* Jacket
* Several pair of underwear
* Several pair of socks
* Robe and Slippers
* Preshrunk 100% cotton fabrics
* Pajamas

* Delicate fabrics
* Leather fabrics
* Dress Clothes
* Wool
* Silks
* 50/50 cotton acrylics
* 100% acrylics
* Any item that may shrink or fade when washed
